Recognizing those who are building a more vibrant and healthier Manhattan for all.

The Manhattan Community Health Foundation’s Cornerstone of Community Health Award is intended to recognize an individual or family that has demonstrated impactful leadership to promote community health through inspiration, innovation, and collaboration.

As a catalyst for a healthier Manhattan, we are committed to improving lives by investing in innovative solutions, building strong partnerships, and championing equitable healthcare access for all. This award celebrates a community member who embodies these core values and is truly a cornerstone of our community’s well-being.

Award Criteria: We are seeking nominations for individuals or families whose work aligns with the following criteria:

  • Investing in Innovative Solutions: The nominee has created or led a new project, service, or approach that addresses a significant health challenge in an innovative way. They are a catalyst for positive change.
  • Building Strong Partnerships: The nominee has a history of collaborating with local organizations, businesses, or other community members to achieve their goals, understanding that improving community health is a collective effort.
  • Championing Equitable Healthcare Access: The nominee’s work shows a strong commitment to health equity, focusing on improving the health and wellness of underserved or vulnerable populations.
  • Inspiration and Role Model: The nominee’s actions and dedication serve as an inspiration to others, motivating them to get involved and contribute to a healthier environment.

How to Nominate: To nominate a deserving individual or family, please complete our online nomination form. The form will ask for a detailed description of the nominee’s work, evidence of their impact, and any supporting materials. Here is a link to the form.

Who is Eligible? This award is open to individuals or families who live or work in the Manhattan community and whose work has had a significant impact on our residents’ health and wellness.

Nomination Deadline: The deadline for nominations is October 15, 2026.

Selection Process: Nominations will be reviewed by the Manhattan Community Health Foundation’s board of directors, who will make the final decision. The award recipient will be recognized annually at the Crystal Gala.
